Your First Visit:
We accept most insurances and are preferred
providers (PPO) for
MetLife, Delta Dental, Cigna, and United Concordia. For
uninsured patients we are able to offer a variety of payment
options and payment plans.
(Note: We are not or do not affiliate with any other web
based insurance providers other than the above mentioned)
Payment is due at time of visit. If
you have insurance, your co-pay and any deductibles are
expected at this time as well. Should you have questions as
to what you are expected to pay please contact the office
where you are scheduled and speak with one of our staff.
All of our forms require
Acrobat Reader which is a free download. By having Acrobat
Reader you will be able to fill out our forms on your computer
and then print them (note: depending on your connection speed
forms may take a couple of seconds to open)..

We take great pride in offering comprehensive quality care for every
patient. We are careful in scheduling your appointments so that
you receive the recommended treatment in a reasonable amount of time
while still trying to accommodate individual needs. It is also
important for our patients to be on time for their scheduled
appointments so that we can keep our schedule running smoothly.
Dentures and Implants Patients:
Information regarding this area of
treatment will be added soon. Should you have questions in
reference to this type of dental care you can contact us at either of
our Colorado locations and we would be more than happy to answer any
questions that you may have.
Adult First Visit:
Be sure to arrive 10-15 minutes earlier than your scheduled
appointment (to save time download the New Patient Registration.)
Please bring your insurance card(s) with you. If you do not have
insurance, please be aware you are responsible for the cost for your
visit.
If you are brining x-rays originals or copies from another
provider, please keep in mind that we use digital x-ray imaging and
in many cases these are not as clear and therefore do not show a
truer image of your oral health. Our doctors may request an
additional x-ray (even if you brought your own). If your x-rays are
older than six months we will require new x-rays. Making sure that
we have the best up to date information is the only way that our
doctors can provide you with the best possible treatment options
while relying on the latest technology.
Child First Visit:
Be sure to arrive 10-15 minutes earlier than your child's scheduled
appointment (to save your time download the New Patient Registration.) Please bring insurance card(s). Also, please
bring only those child or children that are scheduled for an
appointment. By doing this it will help in making your child's
visit more pleasant.
We do allow you to accompany your child; however, do to space
limitations and that affecting the ability of our staff to
adequately perform their duties, we only allow one guardian back per
child being treated at a time.
Children that are of legal age to drive (16 to 17 years of age)
can be treated without the guardian being present; however, we
require a written Consent for treatment from a legal guardian.
Children that are 15 years and under require a legal guardian
being on site during treatment. If you or a legal guardian can
not come on the day of your child's visit, you can assign a
Proxy Consent for
Treatment.
